We empower individuals with disabilities to understand and exercise their rights, make informed decisions, and advocate for themselves in all areas of life. Our advocacy services provide guidance, resources, and support to help individuals overcome barriers, access services, and achieve greater independence. We also work to promote systemic change that improves accessibility, inclusion, and equal opportunities within our communities.
Independent Living Skills Training helps individuals develop the knowledge and confidence needed to live as independently as possible. Training is tailored to each person’s goals and may include budgeting, meal planning, transportation, communication, self-advocacy, household management, and other daily living skills. Our goal is to provide practical tools that support greater self-sufficiency and personal choice.
Finding the right resources can be challenging. Our Information & Referral services connect individuals, families, and caregivers with community programs, benefits, services, and supports that meet their unique needs. Whether you’re looking for housing resources, healthcare services, transportation options, employment supports, or other community resources, we’re here to help you navigate available opportunities.
Peer Support connects individuals with others who have shared lived experiences with disability. Through one-on-one conversations, support groups, and mentoring opportunities, participants can build relationships, gain encouragement, share knowledge, and learn practical strategies for independent living. Peer Support fosters confidence, community involvement, and a sense of belonging.
Our Transition services support individuals as they move toward greater independence and community participation. Community Transition services help individuals relocate from nursing facilities or other institutional settings into homes within the community.
Youth Transition services assist young people with disabilities as they prepare for adulthood, including education, employment, independent living, and community engagement. We work alongside individuals and families to create a successful path forward based on each person’s goals and aspirations.
In addition to our core Independent Living services, we offer a variety of supports designed to help individuals with disabilities remain independent and connected to their communities. Services may include case management, minor home accessibility modifications, assistive technology resources and our loaner equipment closet, ADA training and technical assistance, disaster case management, emergency transportation coordination, and assistance with public benefits applications and appeals. These services are tailored to individual needs and help remove barriers to independence, accessibility, and community participation.
